Trivita Health Center Policies: 


Insurance:

We bill participating insurance companies as a courtesy to you. Services are provided with the understanding that you (not an insurance company) are ultimately responsible for the cost of the services that you are scheduling. Insurance plans vary and not all services may be covered. You are responsible for knowing what services are covered by your particular plan, please check with your plan by calling the number on the back of your card before your appointment. If you need a prior authorization for services, this must be completed before you are scheduled. If you fail to obtain authorization, any and all charges incurred and not reimbursed, will ultimately be your financial responsibility. If you have a change in your insurance, it is your responsibility to notify the office as soon as possible to ensure appropriate billing.


We are currently in-network with the following insurance companies: 

Optum/United Health, Cigna, Aetna

Out of Network Benefits: 

We can offer an itemized receipt of payment and the services provided regarding the possibility of reimbursement via out of network insurance. Most major insurance carriers will provide adequate reimbursement if out of network. Please feel free to inquire when scheduling your appointment.

Payment:

Payment is expected at the time of your or your appointment. This may include deductibles, co-pays, uncovered services, or any balance due on your account. Please understand it’s your responsibility for services rendered. If you are not in a financial position to pay at the time of the appointment, we will be happy to reschedule the appointment for a more convenient time. Outstanding balances of greater than $100 and/or more than 60 days must be paid in full or a payment plan set up and adhered to before any new appointments will be made or refills for medications given. After 90 days, we reserve the right to send your bill to collections if no prior agreement has been made. If your balance has been sent to collections, you will be responsible for all additional collection fees.

Fee Schedule: 

Payment of fees is due at the time of service by credit card. A valid credit card number is required to hold first and one-time appointments. Our office accepts select insurance plans, so please call to verify if we're in-network with your insurance carrier. 


Telephone Fees

Fees may apply to telephone conversations with family members, therapists and other providers. There is no charge for emergent calls or discussions about routine matters such as appointment scheduling. Matters requiring more than 10 minutes require face to face discussion. Please note that while there are billing codes for these sessions, most insurers do not honor them.


Administrative Fees

The provider (Dr. Newman) is a solo practitioner and does not employ staff for tasks such as obtaining pre-authorizations and calling in refills. Therefore, he may need to spend a significant amount of time between visits attending to these matters on your behalf.

Typed letters/Authorizations: $50.00

Fee level may vary depending on the breadth of the documents. It is reasonable to assess the same fee as consultation fees when the document is time consuming (i.e. 1 hour = $250).


Missed Appointments

You will be charged a $100.00 no show fee if you miss or cancel with less than 48 hours (2 business days) notice. You will be notified the business day prior to your scheduled appointment to confirm attendance or need to change appointment. Please remember this is an automated reminder and does not constitute an excuse for a no-show fee exemption.  


Prescriptions

Office visits are required for all new prescriptions. You are responsible for keeping track of remaining refills and schedule appointments accordingly.

*Cash Pay or Out of Network with medication management, Integrative & Functional medicine and/or psychotherapy

60 minute new patient visit $250.00

30 minute follow up: $150.00

Late Cancellations / No Shows:

If you arrive more than 10 minutes late to an appointment, it will be considered a missed appointment and must be rescheduled. If you need to cancel an upcoming appointment for any reason, please allow at least 24-hours notice, not including weekends and holidays. The late cancellation/ no-show fee for a missed appointment is $100. Please note that insurance will not cover this and payment must be received before your next appointment is scheduled.

If you fail to show to a scheduled appointment and do not call within 30 days to reschedule, you will be considered to have terminated treatment. All patients must be seen at least one time every 3 months. If a required follow up is not made at the three-month period you will no longer be considered an active patient of the clinic and we will no longer accept responsibility for your care.

Laboratory Testing: 

The provider preferentially orders tests from laboratories that will accept and bill through your traditional medical insurance coverage. There may, however, be additional outside lab testing needed at times, but of course all lab costs will be discussed with you, and every effort is made to code the requisitions properly in order to maximize your insurance coverage.

Medication Refills:

Medication refills are done during appointment time only so that your response to the medication(s) can be properly assessed. Prescriptions will be written with enough refills to last until your next appointment. We recommend scheduling your next appointment at the end of each session so that we can be sure you are following your treatment plan, in addition to still having medication left. These strict policies primarily reflect our concern for our clients’ well-being. Self-assessment of psychiatric symptoms can be difficult and requires quite a bit of practice and feedback, especially early in treatment. We recommend scheduling your next appointment at the end of each follow-up appointment as our schedule fills up quickly.

If something unexpected occurs and you run out of medication before your next scheduled follow-up, please call or message our office ASAP to request a refill. Please allow up to 5 business days for the refill to be approved by insurance, if appropriate.

Controlled Substances:

The provider (Dr. Newman) only prescribes Controlled Substances such as stimulants, hypnotics and benzodiazepines in California. Please note these are highly controlled medications which need proper monitoring that need to be carefully dispensed.  

Court Evaluations and Legal Matters:

Trivita Health Center and its' provider does not provide court evaluations or court testimony. We are happy to refer you to someone else that has this type of training and is willing to to accept your case.

If we are ordered to testify in proceedings, it seriously undermines our therapeutic provider-patient relationship, is very disruptive to the office routine, and is unfair to our other patients. Please inform us immediately if you are involved in, or plan to go to court. We accept clients only with the specific agreement that that they will not involve Trivita Health Center in any legal matters, including but not limited to child custody, court-ordered treatment, worker’s compensation claims, and criminal cases. We do not provide legal opinions, evaluation, nor testify for disability or child custody cases.
